Welcome to the home page of the HCF Healthy Weight For Life (HWFL) program.
Aim of Healthy Weight For Life (HWFL)
The overall aim of the HWFL program is to help overweight HCF members, in collaboration with their healthcare team, to lose weight, improve their health status and learn the lifestyle skills that will help them stay in control of their weight for life (AT NO CHARGE TO ELIGIBLE MEMBERS - see criteria below).
What is the Healthy Weight For Life (HWFL) program?*
The Healthy Weight For Life program is a unique step by step treatment process that is delivered in 3 clearly structured phases over 18 weeks. The program is run by Prima Health Solutions (PHS) for and on behalf of HCF.
The program combines:
- KicStart™ VLCD - independently researched very low calorie diet meal replacement
- TEMplate System™ - dietitian developed, eating and lifestyle modification program
- Online Education Modules - 9 educational presentations and downloadable information sheets
- Personalised Online health tracking form
All the material you need to complete each phase of the program will be couriered directly to your nominated street address.

Who can participate?*
The program is available to HCF members who :
- have Type 2 Diabetes (or are at risk of developing type 2 diabetes)
- have a body mass index (BMI) ≥27
- are 25 years of age or older
- have an HCF hospital product that they have held for at least 12 months
- obtain approval from their medical practitioner to participate
- maintain medical supervision for existing medical conditions
- are willing to provide their progress data over the course of the program (including weight, waist circumference, HbA1c, blood pressure and cholesterol)
